Care Planning


CareMate begins by meeting with the older adult in their home and assessing their functional, cognitive, emotional and social needs. The assessment is designed to get a comprehensive picture of the older adult's needs and desires. Once the assessment is complete, creating an individualized plan of care to meet the older adult's needs is the next step.

The plan of care focuses on what type of assistance the older adult needs to function at their optimal level and to keep them as independent as possible. One of the most important aspects in planning care is working with the older adult to develop the plan of care. The assessment is valuable, but the older adult must agree that the results are consistent with their perception of care needs. The family may be involved with the planning as well. The goal of the plan of care process is to develop a program that is acceptable to all involved.

Once a plan of care is developed that reflects the needs of the older adult, it needs to be put into action. Coordinating, arranging and monitoring the assistance needed, as determined in plan of care, can be done by the geriatric care manager or the family.

A plan of care reflects the needs of the older adult at the time of the assessment, but the older adult's needs may change over time. It is a good idea to keep the geriatric care manager involved to help ensure the plan remains appropriate. The geriatric manager also can provide early intervention for any problems that may arise and assist with any changes in the older adult's care needs.

CareMate's goal is to help eliminate or postpone inappropriate institutional care and to utilize local services and resources. As a result, time and money can be saved for the long distance caregiver who must forego work and family to coordinate, arrange and monitor care for a loved one.
