Elder Care Planning Services
is a private service offered in Monroe
County, Wisconsin and the surrounding
area to meet the needs of older adults
and their families.
CareMate's Service Area by City:
Sparta, La Crosse, Tomah, Cashton,
Viroqua, Westby, Warrens, Onalaska, Black
River Falls, Cataract, West Salem,
Holmen.

adults have always had a big influence in
my life. I find working with them to be
very fulfilling. My name is Sheri Fanning. I
am a registered nurse with a
certification in Gerontology. I live in
Sparta with my husband and 3 daughters. I
have worked in the health care field for
14 years. During the past four years, I
have specialized in home care.
Many families
struggle, not knowing when or if a loved
one needs assistance to remain in their
home environment. Family members often do
not know whom to turn to for help in
making personal care decisions.
Unfortunately, it sometimes takes a
crisis for families to realize a loved
one needs some extra support. My goal is
to help families before, during and after
a crisis.

has been certified as an
Advanced Directive
Counselor.
Sheri was nominated as
Outstanding Woman of the
Year 1999 by the Sparta
Women's Association.
Sheri is an active
advocate for senior
citizens. |
